Pharmaceutical companies are developing new treatments for the common bacterial diseases such as acne, rosacea, and pneumonia, which are among the most common types of bacterial infections among the Australian population. Doxycycline is the first line of treatment for these infections in Australia, with a wide choice of brands and formulation, which has resulted in better outcomes than other antibiotics, such as tetracycline and doxycycline. However, there is limited data on the effectiveness of doxycycline for these diseases, with only two studies that reported results that are of interest. This study aimed to evaluate the effectiveness of doxycycline for bacterial infection caused by rosacea.
This is a retrospective study of 637 Australians with rosacea and 795 patients with acne who were treated at the Department of Dermatology, Sydney Hospital. The sample size was calculated using the Poisson model, and the data were analysed using Poisson regression and logistic regression. The inclusion criteria included patients with rosacea and patients with acne, and these were treated using doxycycline or tetracycline alone. Outcomes included the following: (i) antibiotic sensitivity and effectiveness (p<0.05), (ii) treatment time, (iii) duration of illness, and (iv) adverse events.
Out of 637 patients with rosacea, only 38 patients (3.5%) were treated with doxycycline alone (p<0.001). Of the patients with acne, 67 patients (14.5%) were treated with doxycycline and 66 patients (14.5%) were treated with tetracycline alone (p=0.02). The proportion of patients with acne was significantly higher with tetracycline alone (45.9%) than with doxycycline alone (22.2% vs. 12.8%) (p<0.01). The proportion of patients with rosacea and acne were significantly higher with tetracycline (50.3% vs. 21.7%) and doxycycline (22.6% vs. 13.4%) than with doxycycline alone (p=0.01 and p<0.05, respectively). The proportion of patients with rosacea and acne with tetracycline treatment was significantly higher with tetracycline and doxycycline combination (23.3% and 21.3%, respectively) than with doxycycline alone (p=0.02 and p=0.01). The proportion of patients with rosacea and acne were significantly higher with tetracycline and doxycycline combination (16.2% and 21.2%, respectively) than with tetracycline alone (p=0.01 and p=0.01, respectively). The treatment with doxycycline and tetracycline combination was more effective than doxycycline alone in reducing inflammatory markers and improving the clinical outcome in rosacea.
This study demonstrates that doxycycline is effective in treating rosacea, which is one of the most common bacterial infections among Australian patients. The study has provided a good picture of the effectiveness of doxycycline in treatment of rosacea and acne, which is an important clinical issue in Australia. The findings of this study are useful for healthcare providers to guide the choice of treatment of rosacea in patients with acne.
